The Medical Social Worker with Carilion Clinic will be an integral part of the care team, contributing professional expertise to support the holistic health of the patient and support for family and/or other caregivers. In collaboration with the health care team the social worker works with the team of health care providers to create an interdisciplinary plan of care identifying needs, strengths and recommended treatment or support strategies. The social worker provides education and support, and makes referrals for other services per the interdisciplinary treatment plan. The social worker presents cases for review at the weekly team meetings and keeps up-to-date-records using EPIC to enter information into the electronic health record. Tasks can include checking the prescription drug monitoring system for the physicians, securing patient informed consents and supervised medication agreements and team with external care providers, peer recovery specialists, and other agencies as relevant to achieve patient treatment outcomes.
Education: Bachelor's Degree in social work or related degree required.
Experience: Social work experience in health care setting preferred. Working for Med/Surgical units as well as Emergency Department, Pediatrics and Birthplace. Must be willing and able to work with substance exposed Newborns and their Families. Team work is a must.
Licensure/Certification\: None required.
Other: Must successfully complete orientation and competency validation for position. Demonstrates critical thinking and creative problem solving skills to move patients effectively and efficiently through the continuum of care. Ability and willingness to work as an integral member of a multi skilled team.
