Carilion Clinic Clinical Coordinator - RN in Roanoke, Virginia
Advanced Illness Care Coordinator position
Hospice or Palliative Care experience strongly preferred. Position includes telephonic care coordination as well as patient home visits. Some travel required around the Roanoke area. Will work with interdisciplinary team to provide home based palliative care and provide advance care planning services.

Education: Graduate of an accredited School of Nursing required. Bachelor’s degree Preferred.
Experience: Minimum of 3 years RN experience required. Hospice and/or Palliative Care expereince strongly preferred.
Licensure, certification, and/or registration: Current VA RN license required. Must successfully complete orientation and competency requirement. BLS required.
Valid Virginia, or state specific, drivers license required for employees working in Home Health or departments where the work requires traveling.
Other Minimum Qualifications:
Effective interpersonal, communication, and team-working skills required. Strong critical thinking, conflict resolution, organization and effective judgement abilities required. Must successfully complete orientation and competency validation requirements. Must be able to meet essential job functions. Designated areas may require Knowledge of Medicare/Medicaid and other third party payor regulations
