The primary role of the PRN Case Manager is to help coordinate admissions, during stay, and after-care needs of each patient based on recommendations of the clinicians and patient’s goals. This is a person-centered process which is driven by an individualized treatment plan that functions to maintain the individual’s well-being and prioritize treatment goals and objectives. The Case Manager will familiarize him/herself with community resources and draw upon them through linkages and referrals to assist patients in accessing the network of resources that will help maintain their community and promote further growth to maximize functional independence.

The Case Manager completes all processes, procedures, and case management activities directly related to admission, treatment and discharge from the Acute program. The Case Manager will work collaboratively with the Director of Clinical Services to ensure timely communication with stakeholders and referral sources. They will ensure all documents are sent to the appropriate team or agency and all follow-up appointments are annotated on the patient’s discharge instructions. The Case Manager also works with the clinicians, nurses, psychiatrists, patients, families, and community agencies as needed, with the goal of linking patients to appropriate continuing care resources. The Clinical Case Manager acts as a member of a multi-disciplinary treatment team, and serves as an advocate for the patient.

The Case Manager shall function in compliance with all organizational policies and observe ethical and confidentiality standards at all times. This position will work closely with members of the therapeutic treatment team, external agencies, clients, families, and members of other departments.

Qualifications

Licensure/Certification/Registration:

    • CPR Preferred (offered on-site)
  • Handle With Care Preferred (offered on-site)

    Education/Experience:

    • Minimum of Bachelor's Degree in Human Services or related field.
  • Minimum of two years experience in healthcare setting preferred.

  • Extensive knowledge of CSA, DSS and/or Virginia System of Behavioral Healthcare preferred.

    General Requirements:

    • Must be at least 21 years of age per Commonwealth of Virginia Interdepartmental Regulation of Children’s Residential Facilities. (22VAC42-11-380, pg. 43)
  • Must be able to multi-task and work at a fast pace.

  • Must have patience, tact, and a cheerful disposition and enthusiasm.

  • Good communication skills; must be able to read, write, speak, and understand the English language.

  • Basic clerical skills, basic experience with office equipment (telephone, copy machine, fax, etc.)

  • Experience with Microsoft Office Software (Word, Excel, PowerPoint, Outlook, etc.)

  • Ability to understand and carry out oral and written instructions.

  • Ability to problem solve and accept feedback when circumstances warrant such actions.

  • Willingness to interact with patients with varying levels of cognitive functioning.

  • Travel may be required.
